Do you know someone
who has made a difference?

Do you know someone who has performed an extraordinary act of courage to save someone's life? Or maybe you know someone who commits countless hours to a special cause?
The Red Cross wants to help you honor and celebrate these individuals at one of its annual Heroes Breakfast Celebrations. The Heroic event must be ongoing or have occurred between January 1, 2009 and December 31, 2009. Nomination forms must be received by December 31, 2009 to be considered. The Hero must live or work, or the event must occur, in one of the towns served by the Connecticut Chapter. Nominations may be made for both living and posthumous Heroes.
The Red Cross touches lives in our state everyday through programs and services such as Disaster Relief, Service to the Armed Forces, Preparedness, and Health & Safety Training. As an organization committed to helping people prevent, prepare for and respond to emergencies in our communities, the Red Cross is honored to salute individuals who make a difference.

Nomination Guidelines:
· Hero must live or work in Connecticut.
· Heroic act must have occurred between January 1, 2009 and December 31, 2009.
· On a separate piece of paper please explain the heroic act or actions.
· Please include reports, news articles, names and phone numbers of witnesses who can verify the actions.
· Please explain why you are nominating this person(s) and how their action(s) exemplify the humanitarian mission of the Red Cross.
· Please complete the information on this form and attach a copy to your nomination.
· All nominations must be received by December 31, 2009.
